Our Mission

Appointments

Couple_1Appointment Reminder Calls: As a service to our patients, we provide a courtesy appointment reminder call and possibly other important calls that may be placed using a prerecorded message. By providing your cell phone number, you consent to receiving such calls at this number.

Appointment Times: Check in is 15 minutes prior to your scheduled appointment time. This is to allow us to collect information from you and assemble your clinical information prior to your appointment. This allows us to optimize the time spent with your provider. We realize your time is valuable and strive, whenever possible, for you to see your provider on time. If you have an unexpected delay, call us as soon as possible and we will try to work with you. However, you may be asked to reschedule your appointment for another day and time.

No Show Policy/Late Cancellation Policy: Failure to keep or cancel your appointment more than 24 hours prior to your appointment time will result in a $25 fee. This fee is due and payable prior to scheduling future appointments. If you have three no show/late cancellations within a 12 month period, you may be discharged from our office. We want to be reasonable and we understand issues beyond your control may prevent you from keeping your appointment time. Please contact your receptionist if that is the case.

Prescription Refills

  • If you need a written prescription, please leave a message on the refill line with the requested information. Allow 24 – 48 hours for your request to be processed.
  • For routine refills, please contact your pharmacy. They will fax us with all the necessary information. Please allow 24 – 48 hours for your request to be processed.  

Your doctor may request an office visit if he has not re-evaluated the medication within one year.  No narcotic or controlled substance refills will be made after clinic hours.  
We will also no longer be writing or refilling controlled medications without an appointment.  
Controlled medications include (but are not limited to): 

  • Pain medications such as Hydrocodone (Norco, Vicodin), Oxycodone (Percocet, Oxycontin), Morphine (MS ContinDuragesic), Codeine, and Tramadol (Ultracet). 
  • Benzodiazepines sometimes used for anxiety such as Xanax (Alprazolam), Ativan (Lorazepam) and Klonopin (Clonazepam). 
  • Sleeping medications such as Ambien (Zolpidem), Restoril (Temazepam), and Lunesta.
  • ADD medications such as Ritalin (Methylphenidate), Adderall (Amphetamine/dextroamphetamine), and Concerta.

If you need a prescription or prescription refill for one of these medications, you will need to schedule an appointment with your primary care.  If you take these medications routinely, please schedule your appointment in advance so that you do not run out of medications prior to seeing your PCP.  We will not be able to refill your medication prior to your appointment with your PCP.

Laboratory Results

If you had a test or procedure today, you will receive the results by mail, email (if you signed up with WebView), or we will call you within 14 days. Please telephone the clinic if you have not heard from us within 14 days.
